Can Working Out Cause Gynecomastia? 

Exercise itself does not cause gynecomastia, but certain factors associated with gym training may contribute to or worsen it. 

1. Anabolic Steroids and Performance-Enhancing Drugs (PEDs) 

One of the leading causes of gynecomastia in bodybuilders and athletes is anabolic steroid use. Many performance-enhancing drugs convert excess testosterone into estrogen through a process called aromatization. This hormonal shift can stimulate breast tissue growth, leading to gynecomastia. 

2. Overtraining and Hormonal Imbalances 

Excessive exercise without proper recovery can increase cortisol levels—the body’s primary stress hormone. Elevated cortisol can disrupt testosterone levels, sometimes leading to an estrogen-dominant state that contributes to gynecomastia. A balanced workout routine with adequate rest and recovery is essential to maintain hormonal stability.

Common signs include:  

Firm or rubbery lumps under the nipple area 

Tenderness or sensitivity in the chest
